When NMR systems work well, they show us where our work processes are weak, both in safety and getting the job done efficiently. But often, organisations just see near miss reports as a bureaucratic formality and miss the chance to leverage NMR to identify root causes of issues. This means the same issues keep coming up, and managers end up using quick fixes or telling workers off instead.
